New Patients: Payment is due in full at the time of service for all initial exams and X-rays. As a courtesy to our patients with insurance, we will file insurance claims for your reimbursement at no charge.

Payment in Full (services under $300)
Payment in full is requested at time of services for treatment under $300.00. If you have insurance we will file your insurance to reimburse to you directly.

Payment in Full with 5 % cash allowance (services over $700)
A bookkeeping courtesy of 5% is given for direct payment in full by cash or check only on the day of treatment, resulting in a one-time payment . (INSURANCE REIMBURSES INSURED DIRECTLY).

Office Payment Plan ($300 - $700.00)
For your convenience, ½ of the estimated portion is due on the day of treatment and the balance is due in full within thirty days of the date of treatment. (INSURANCE REIMBURSES DOCTORS OFFICE DIRECTLY).

Office Payment Plan (over $700.00)
For your convenience, ½ of the estimated portion (after Insurance) is due on the day of treatment and the balance may be paid in 2 equal payments over the next 2 months beginning thirty days from the date of treatment. (INSURANCE REIMBURSES DOCTORS OFFICE DIRECTLY).

Insurance:
Your treatment plan will include a breakdown of all fees and will inform you of all costs before treatment is rendered. Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary greatly, we can only estimate your coverage in good faith and cannot guarantee coverage. We do not guarantee that your insurance company will pay for all of the treatment necessary to restore dental health. Specific exclusions, omissions and contractual limitations that are written in your policy often limit the amount of coverage that you should expect to receive. Our staff will help you receive the maximum benefits available under your policy. However, please remember that you’re fully responsible for all fees charged by this office regardless of your insurance coverage.
